SureVoIP Number Porting Form
This guide is relevant to the SureVoIP Number Porting Form.
This form is to be used for porting UK geographic numbers beginning 01 and 02.

Porting Form Explained
Section One: Customer Information
This section requires you to fill out your contact details. This is so we can locate your account on our systems and so we can contact you regarding this order.
Section Two: Installation Details
This section requires you to provide us with the address your service is currently being provided on.
This is generally one of the following
- Your address where the lines are physically provided
- If service is already VoIP, then it will be the address they hold on file for you
- If the numbers are forwarding all calls from the BT exchange then you will need to provide us with the name of the BT exchange and the exchange post code.
This can be obtained by contacting your current provider.
It is imperative that this section is filled out accurately. If the wrong address is provided then a porting rejection charge will be incurred.
Section Three: About Your Current Telephone Service
This section requires you to tell us a little about your current provider and what services they are supplying you with.
The key elements are:
- Your current provider
- Your name or the account holder name
- The service being provided (ie Single Line (analogue line), ISDN, Feature Line etc)
- If the line is Single Line or Multi Line
- If RedCare and/or broadband is being provided
- How many lines (channels) are being provided. More specifically, how many callers you can have at any given time to your numbers.
It is imperative that this section is filled out accurately. If the wrong line type is advised, or RedCare is on the line, then a porting rejection charge may be incurred.
Section Four: Numbers To Be Ported
This section asks you to tell us about the numbers you wish to port and any other numbers you wish to cease.
Main Billing Number is the primary number on your account. If you have more numbers, such as DDI ranges or associated numbers then these will link to the Main Billing Number.
If your installation is a Multi Line installation then you will need to correctly identify the MBN. If you are unsure which number is your MBN then you will need to contact your current provider to confirm.
Are there other numbers associated with the installation that you wish to port? Put in any DDI ranges or associated numbers here. What you put into this box will be ported to SureVoIP.
It is imperative that this section is filled out accurately. If you provide us with the wrong Main Billing Number or miss out DDI ranges or associated numbers, then a porting rejection charge may be incurred.
Section Five: Numbers To Be Ceased
Put in any DDI ranges or associated numbers here that you would like to cease on the porting date. This can be left blank if you do not wish to cease any numbers.
Section Six: Porting Date Required
This can be left blank and we will aim to port your numbers at the earliest possible date.
If you have a specific date in mind then please feel free to specify this here. Please note the minimum lead times for your particular line type.
Section Seven: Customer Authorisation
Please read the contents of this section as you will be deemed to have accepted these terms once you sign the box and submit to us.
